Support Coordination

To further enhance our supports to people with complex needs, Prag Consulting also offers a range of Support Coordination services including:

Coordination of Support (Cos)

Cos is designed to assist participants to achieve their goals. Goals may be NDIS related, for example, to connect with providers, or related to mainstream or informal supports. Coordination of supports can assist participants to access employment or education services or to connect with family, friends and community. Consistent with Prag Consulting’s philosophy and practice, Support Coordinators work with participants and supporters to identify what is most important and the supports required to meet identified goals.

Specialist Support Coordination (SSC)

SSC is the provision of support that is complex and high risk. SSC is often provided to support participants who are accessing services across multiple areas (for example: NDIS, as well as mental health or justice or child protection). Prag Specialist Support Coordinators providing SSC have qualifications, skills and experience in support participants with complex needs. Specialist Support Coordinators work with participants and supporters to identify needs, develop meaningful goals, and ensure supports are in plan to manage challenges and risks to participant safety and well being.
